In order to accommodate everyone, please ensure that you arrive on time for your scheduled appointment

It is recommended to complete the Intake form before-hand so we can have more time for assessment and treatment, if you have problems with the online Intake form one can be downloaded here: Intake form

Masks or face coverings are optional as of March 2022

Appointments not cancelled or rescheduled 24 hours beforehand will be subject to a no-show fee of 75% of the session rate.

Toronto office has been moved as of July 1, 2023, to our new location near the Cookie factory by Bermondsey and O'Connor:

Building 2 is located within the plaza at 1450 O'Connor past the Tim Hortons and the medical building.
Parking is free!
To access the office please use the left entrance of Building #2 (between Fitt bike and Uhaul/The Boxe shoppe) and enter the second door to the left - following the passageway to the end of the corridor.

Candice's home has been open to patients from the Durham region and surrounding areas since 2019.

You are welcomed to park on the driveway - preferably on the left / southside of the driveway.

There will be room for 1 visitor to accompany the patient during sessions.

Respectfully, as this is Candice's home please stay home if you are sick / ill. The appointment can always be rescheduled if notice is given before the appointment time.
